A clearer starting point for a difficult family conversation.

You do not need to know the name of a care package before getting in touch. Begin with what is changing at home, what matters to the person, and which parts of the day now feel less manageable.

Four useful things to think through.

Which part of the day needs support?

Morning personal care, preparing food, getting to an appointment, companionship, an evening routine or support through the night can each point to a different shape of care.

What does the person want to keep doing?

The starting point is not only what someone finds difficult. Note the routines, choices, relationships and activities they want care to protect.

Has anything changed recently?

A hospital stay, fall, diagnosis, change in mobility or increasing pressure on a family carer can help explain what support is needed now.

Is transport part of the need?

Make the practical details visible.
